An exciting opportunity has become available for a dynamic Band 7 nurse or paramedic to join our Consultant led Virtual Ward team.
The Virtual Ward is a new and innovative integrated service with the aim to support patients to receive the care they need at home, rather than in hospital. There is growing evidence that demonstrates patient, system and public benefits to the Virtual ward model.
Central and North West London NHS Foundation Trust works in partnership with Central London Community Healthcare NHS Trust and Royal Free London NHS Foundation Trust to deliver an acute care service within the community named Rapid Access Services.
Additionally, the Virtual Ward service (formed in December 2021) will now also be provided via the Rapid Access Services. Patients are admitted to the Virtual Ward as a form of early discharge from the local acute trusts while still under the care of a Consultant. This service offers a mix of conditions but will include patients requiring IV medication; Daily observations; Delirium care; At home suctioning support (Yankeur); and as the service grows, the presenting complaints accepted into the service will increase.
Main duties of the job
The post holder will act as a core member of the daily virtual ward round, contributing clinical nursing expertise to MDT discussions, while actively advocating for our patient's needs
Liaise effectively with the MDT i.e. medical staff, therapists, pharmacists, community partners and patients to develop holistic care plans and coordinate provisions.
Maintain accurate, timely, and professional clinical documentation
Use an array of technology to support your day-to-day work such as; Doc Abode, Electronic patient records, Teams, Skype, etc
Conduct comprehensive clinical assessments, drawing on advanced physical assessment training and the latest evidence-based practices. You should demonstrate competence and confidence in formulating diagnoses, including consideration of differential diagnoses
Interpret clinical data including vital signs, NEWS2 scores, pathology results, and symptom progression to guide safe patient management
Support early recognition of deterioration and escalate appropriately to medical colleagues or emergency services when required
Promote and uphold clinical excellence, supporting evidence-based practice and ensuring all care meets professional and regulatory standards

This is because of the high-acuity nature of the ward / community environment and the need to ensure the safety of both patients and staff. The role is conditional upon candidates being able to engage in TMVA training and resus training during the course of their employment whilst undertaking this role.
