Our North hub is looking for both to deliver high quality, evidence-based nursing interventions to our patients in the highly diverse areas of Kensington and Chelsea.

Main duties of the job
- To be accountable for the care and support of a defined caseload of adults of working age.
- To screen for health and social care needs (including accommodation, finances, and support in accessing education, training or employment) and negotiate care plans with service users and their carers, remaining objective when working with diagnostic uncertainty and undertake assessments of need (in compliance with the Care Act 2014).
- To undertake risk assessments and develop robust risk management plans, according to Trust policy.
- To safely administer and store medication as needed in the community and ensure that relevant Pharmacy guidelines and policies are adhered to.
- To monitor treatment, support concordance and ensure prompt action is taken to alleviate unwanted effects of prescribed medication.
- To ensure delivery of care plans in partnership with multi-disciplinary colleagues and a range of health and social care agencies, within the DIALOG+ framewor
Please note : this role requires successful candidates to be able to engage in Therapeutic Management of Violence and Aggression (TMVA) training, including the use of force techniques and the ability to attend resus training and perform CPR. This is because of the high-acuity nature of the ward environment and the need to ensure the safety of both patients and staff. The role is conditional upon candidates being able to engage in TMVA training and resus training during the course of their employment whilst undertaking this role.
