This is an opportunity for qualified nurses who are looking to develop / establish their skills and expand their abilities to work within the community. In turn you will be able to make a meaningful impact in a different capacity than your existing role. You will need to demonstrate resilience and the ability to adapt to the changing landscape and opportunities. The post holder will play a pivotal role in providing intensive support and treatment to patients in the community in the acute phase of a mental health crisis as an alternative to inpatient care. We are looking as a trust to change the approach as an alternative to inpatient care so that the patient can be in familiar surroundings.

Join our team at HTT, where your work-life balance is a priority! Our services run from 9 am to 5 pm, seven days a week, with no night shifts (some weekend work would be required). This structure not only ensures you have dedicated time for your personal life but also opens up opportunities for you to take on additional bank hours with us or explore roles elsewhere across the trust.

Main duties of the job

This role involves intensive support and treatment to patients in the community in the acute phase of a mental health crisis as an alternative to inpatient care; monitoring patients' safety and well-being to promote independence in the community; acting as gate keepers to admissions; supporting early discharges from hospital.

You will also be expected to work in collaboration with the Operational Team Manager, Clinical Leads/Senior Practitioners and to have close working relationships with the community teams, inpatient services, and the HTT backup across the service.
