Job summary

ARC Hounslow is a service provided in partnership by CNWL and Phoenix Futures as a free and confidential service for people who live in the London Borough of Hounslow.

The service helps people break a cycle of addiction to substances such as heroin, cocaine, and new psychoactive substance as well as long term alcohol addiction.

We work with people at any stage of their alcohol or drug difficulties to provide a single point of access to assessment and treatment for problems.

Main duties of the job

  • Joint working with partner agencies and the local Borough of Hounslow, including West Middlesex University Hospital, Health Centres / GP surgeries, Food Banks, Children's Centres, Job Centre Plus, etc.
  • Face to face re-engagement, support and clinical reviews of clients who are hard to reach or cannot attend the service.
  • Delivering Initial Brief Assessments and referring into ARC service where appropriate.
  • Manage a small caseload of service users with co-occurring issues.
  • Delivering training to local and partner agencies in the Borough of Hounslow.

We are recruiting for an Outreach Worker to facilitate and deliver alcohol and drug interventions in the community and to refer into the clinical service when required.

  • Each service user will be assigned a keyworker who has the responsibility to make sure appropriate care is carried out by themselves and other members of the team.
  • Written and verbal communication between team members will be in place and allow for continuity of care where needed.
  • The post holder will be expected to see service users within Hounslows supported accommodation schemes and engage in support regarding their drug and/or alcohol use.
  • To refer and empower service users to self-refer to Hounslows existing substance use treatment service, some of whom are reluctant to engage and continue to pose significant risk to their health through their substance use.
  • To refer and signpost service users with complex needs including co-occurring mental health issues and experiences with homelessness to appropriate services.
  • The post holder will be expected to attend accommodation schemes on council sites to support service users who are unable to attend the local substance use and harm reduction services.
  • Treatment Outcome Profiles to be used to review service users progress in reducing their drug/alcohol use and changes in their overall wellbeing.

  • Educated to Degree level or equivalent level of experience working with relevant client groups
  • G.C.S.E/A levels or equivalent qualification and experience

Experience

  • Experience of having worked in the NHS, or a similar clinical setting
  • Experience of having worked with substance misuse groups
  • Administering and co-ordinating outreach in a range of settings, including the organisation of different activities, prioritising tasks, monitoring and evaluating
  • Delivery of workshops to a variety of users
  • Experienced in using screening tool such as AUDIT C/AUDIT and delivering brief interventions

Skills and knowledge

  • Excellent organisational & prioritising skills
  • Proficient IT skills Microsoft Office (Word/Excel/Access)
  • Excellent interpersonal and communication skills with range of individuals, including hard to reach client groups namely: Street drinking community, Homeless Clients, Drug Users, BAME
  • Sound knowledge and understanding of the interventions for people misusing substances and barriers to these client groups accessing services
  • Knowledge of current legislation and national guidance relating the client groups targeted
