Main duties of the job
This is a new role and new team within Brent HTT. Outreach team is a sub-team of HTT, with a focus on supporting residents of NW10 and NW2, who frequently present to Accident and Emergency in various hospitals. This role will entail our outreach staff working with the GPs, Psych Liaison, A&E, and other third party sectors in and around London.
Further, the role involves intensive support and treatment to patients in the community in the acute phase of a mental health crisis as an alternative to inpatient care; monitoring patients' safety and well-being to promote independence in the community; acting as gate keepers to admissions; supporting early discharges from hospital.
