Job summary
We are looking for an experience band 6 or band 7 social worker to join out Integrated Discharge Team in Riverside in Hillingdon or Park Royal in Brent.
As a band 6 this will be a developmental role within 6 months to get you ready to be a substantive band 7.
For a band 7 it will be a developmental role in developing skills, knowledge and abilities through multi-agency working with mental health service users in inpatient settings, to remove barriers to discharge.

Main duties of the job
- Working as part of our Integrated Discharge Team.
- Identifying and removing social barriers to discharge.
- Manage a caseload across a few in-patient wards, whilst having support from a band 4 support worker in managing the caseload.
- You will be working with other internal and external agencies E.G: housing, adult social care and immigration.
- You must have an in-depth knowledge of social care legislation and associated local authority guidance.
- You will be part of the local inpatient team and the trust wide Integrated Discharge Team.
- Be in direct and indirect contact with service users, family and carers.
- Supervisor responsibility for unregistered support workers within the team.
- You will work in a targeted way to achieve the discharge plan for inpatients, maximising independence and choice for the individual, and work in a solution focussed way to ensure that any social barriers to discharge are addressed in a timely manner to support an effective discharge.
- Screen new admissions to hospital to ascertain any barriers to discharge and will join key meetings to support with discharge planning.
- This is a hospital based post and requires the successful, applicant to work on site and face to face in multidisciplinary meetings and whilst supporting service users.
- You will focus on all KPI's linked to length of stay and out of area placement elimination.
