Service Manager to the Integrated Discharge Team in CNWL

A fantastic secondment opportunity has arisen in CNWL's Integrated Discharge Team (IDT) within the Urgent Care service.

The post holder will operationally and clinically lead the IDT service within CNWL's London region. This is an exciting opportunity to join a growing, creative and innovative service and work collaboratively with the Deputy Head of Urgent Care in developing the service.

We are looking for an experienced Social Worker who is already working in a band 7 or 8A or an equivalent grade within the NHS or Adult Social Care to join our team. Perspective applicant should have some management experience of leading a team or service and is looking for more responsibility and a new challenges.

This dynamic service has been running since 2022, offering health and social care input to identify and eliminate barriers to service users discharge.

The IDT Service Manager will have managerial responsibility for the day to day running of the service, reporting directly to and working closely with the Deputy Head of Urgent Care. They will provide senior management and clinical input support to the team, playing a key role in building internal and external multiagency interfaces.

The IDT Service Manager will generally work a Monday to Friday 9-5 work pattern, although may be expected to work flexibly at times dependent on the needs of the service. Generally shift and night working is not required in this role.

Main duties of the job

  • The post holder will work within the Urgent Care Service pathway, providing operational management and clinical leadership to the Integrated Discharge Teams.
  • - Provide clinical oversight, whilst delivering key performance indicators.
  • - The post holder will be the Senior Social work lead providing expert advice on social work legislation, trust and adult social care policy and procedures to unlock barriers so safe and effective discharges progress without delay.
  • - The post holder will work with the Urgent Care Service Senior Management Team to deliver the vision of the Trust to provide a quality led service.
  • -Be an expect in social work, working collaboratively to meet the strategic direction of the service whilst developing the service and team of professionals.
  • - The role, the post holder will have co-operational management for the management of VCSE contracts for the IDT service.
  • - Manage a case load of service users within the IDT framework providing expert clinical guidance to fellow social workers within the service and MDTs during case reviews.

  • To work with the IDT and as part of a multi-disciplinary team to provide discharge planning support where necessary and support case management for the most complex group of patients.
  • Promote referrals into the Complex Care Forum
  • To monitor the VCSE contracts and the adherence to these contracts.
  • To monitor the IDT service objectives and report on deliverance monthly.
  • To support the IDT team to case manage and co-ordinate the discharge plans for patients requiring integrated support in the community.
  • To ensure smooth and timely communication between the Trust and partner agencies involved in patient discharge / transfer of care to avoid unnecessary delays
  • To support the IDT in managing the patient pathway and flow through effective case management, working with an integrated approach with all partners
  • To ensure that systems are in place to monitor stakeholders views of services and ensure that service users, carers, relevant community organisations and other key stakeholders are involved in and influence key decisions on service development and continuous improvement
  • To participate in the evaluation of the services provided within Mental Health Services and actively promote service development including undertaking relevant audit and research.
  • To ensure that IDT contributes fully to the development of the business plan for the Division and assists in the setting of strategic objectives for the Service
