An exciting opportunity has arisen for the Clinical Lead for Westminster Community Mental Health Hub (CMHH). We are looking for a candidate with robust clinical expertise who is also able to demonstrate progressive skills in leadership and service development.
The North Westminster Community Mental Health Hub, based in Woodfield Road is looking for a passionate candidate who is looking to make a difference whilst working in an environment that promotes staff development and training opportunities. CNWL is passionate about developing its own staff, driving them to go on to become more established practitioners. This is a great opportunity to take your next steps in CNWL.
There are 2 Community Mental Health Hubs in Westminster, one North and one in the South, each is aligned to two local Primary Care Networks (PCN). Working in this way allows each Hub to have a core structure and function to meet the needs of the local population, aligned with our primary care colleagues.
The post is open to registered mental health nurses and allied professionals.
Main duties of the job

The successful applicant will take the day to day clinical and team management responsibility at team level using performance management systems to ensure the effective delivery of clinical activity and quality standards to meet service specifications. You will also be expected to work in collaboration with the Operational Team Manager, Clinical Leads/Senior Practitioners and to have close working relationships with the Inpatient Service and HTT.

In conjunction with the team manager:
- To be the clinical team leader for the Community Team, responsible for the clinical practice of the team and ensuring that priorities are met.
- To assist and deputise for colleagues in the Mental Health Hub other sub-team as necessary
- To line manage CPNs, OTs, unqualified staff and to line manage the work of the psychologists.
- To support with ensuring Care Plans are in place including the roll out of the Dialog+ framework for Care Planning
- To work within the relevant legislative frameworks |(e.g. Mental Health Act (1983) Mental Capacity Act (2005), Deprivation of Liberty Safeguards (DoLs), the Care Act 2014 etc
- To promote a culture of service user choice and control within team members daily practice to be monitored and developed through supervision and appraisal
- To ensure that the team works within the relevant frameworks and policies for safeguarding vulnerable adults
- To ensure that services within the multidisciplinary team are well co-ordinated in so that they are responsive and attuned to the needs of its service users

- Foster and implement a multi-disciplinary team approach in all aspects of clinical and non-clinical performance.
INDICATORS:
- Screen and accept referrals and undertake initial assessment in accordance with the Teams Operational Policy.
- Work as a leading member of the multidisciplinary team; and work with other team members, to participate in the formulation, review, implementation and monitoring policies for the service.
- Provide immediate support to patients, relatives and carers.
- Follow up will use appropriate evidence based clinical skills. The post holder will be expected to function during follow up sessions as an autonomous practitioner.
- Provide support and information for significant others involved in patient care, or referral to appropriate agencies.
