As part of our Home 1st service, you will be working with a highly motivated and innovative team to lead on the community inpatient pathway.
As a team we are passionate about putting patients first and working with our multiple stakeholders to facilitate safe discharge to our Seacole Virtual Hospital which you would be leading on for our community. The virtual hospital is set within 4 sites across Milton Keynes and includes our Windsor Intermediate Care Unit which is a 19 bedded rehabilitation unit.
Since the set up of Seacole in 2020, it has gone from strength to strength and has been influential in our approach to placing patients at the centre of all that we do and has resulted in robust partnership working with our stakeholders. Our vision is for integrated, seamless transfers of patient care for our patient groups, high quality patient focused outcomes and exceptional patient experiences.

We are looking for a highly motivated, enthusiastic and innovative leader to oversee the Seacole Hospital and provide high quality clinical and transformation leadership for a fixed term period of 9 months to cover maternity leave.
Main duties of the job
To provide high quality clinical leadership for WICU (a 19 bedded intermediate care unit) and the wider Seacole Community Hospital (51 bedded virtual hospital).
To provide transformational leadership and service modernisation to the pathway.
To enhance the development of evidence based clinical practice for inpatient rehabilitation.
To provide clear operational leadership to the team to ensure compliance, safety and quality at all times.
To be aware of the national guidelines for in- patient care and to implement an appropriate strategy within the unit.
To work with the service manager and the wider MK system partners to review and develop new pathways of care as identified.

PRINCIPAL DUTIES
- Responsible for managing WICU and the Seacole Hospital by ensuring there are safe systems of working that support Patient and Staff Safety, Quality, Effectiveness and Compliance.
- Responsible for ensuring there are robust clinical processes for admission and discharge, management of the patient whilst on the unit and the deteriorating patient.
- To provide clinical and professional leadership to the team.
- To be part of the MDT for the unit, ensuring that patients have the appropriate level of input to optimise their rehabilitation in preparation for returning to their primary place of residence.
- To ensure safe staffing levels for the unit and manage recruitment and retention to support this.
- To work closely with the GPs, Community Services, the Home 1st Therapy, Urgent Community Response and Virtual Ward teams and MKUHFT to ensure appropriate and safe admissions to the unit.
- To work with the Home 1st Therapies and Reablement teams and MKC Social Care to ensure timely supported discharge from the unit to the patients primary place of residence.
- Monitor compliance with KPIs and where necessary develop robust action plans to address non- compliance.
- To assess compliance with CQC standards and where necessary develop robust action plans to bring the unit in line to achieve a minimum rating of Good.
- To appropriately represent CNWL and Home 1st services internally and externally.
