Job summary
CNWL are currently looking for a part time Speciality Doctor (8 PA's) as a Community Psychiatry of Learning Disability . The post holder will work alongside two other full-time Consultant Psychiatrists and one core trainee (CT 1-3).
The post is being advertised in order to recruit following retirement of a colleague.The post would be ideal for an experienced Specialty Doctor who wishes to work with support locally from two other very experienced Consultant Psychiatrists as well as a well-established multi-disciplinary team.
There will be 1 session for personal development which will include training, CPD activities, participation in audit/Quality Improvement (QI) and any requirements for appraisal/revalidation. Relevant CPD is available on Mondays in the Kingswood Centre in Brent and Barts NHS Hospital Trust and there will be an expectation to participate in relevant Royal College of Psychiatry professional development.
Main duties of the job
- To provide a Specialty Doctor Psychiatric service with colleagues into the Enfield Integrated Learning Disabilities Service.
- To ensure, support and develop the provision of ongoing psychiatric input to people with Learning Disabilities living in the community.
- To attend community team referrals, complex cases reviews. This would include taking on a specific lead in one area (to be negotiated depending on interest/experience).
- The post holder can expect to receive approximately 2-3 referrals per month. The caseload of the post is about 100 patients with the LD and various mental health illness or challenging behavior, autism.
- The community service operates a mixture of clinics run in the central base and work in the most appropriate setting e.g. within the residential provision or day care settings. This also includes domiciliary visits. The Doctor is expected to be patient-centered and work flexibly with the multidisciplinary team, families, carers and other professionals.
- To be available within normal working hours, in conjunction with the other Consultants and junior doctors to take responsibility for urgent assessments including formal mental health act assessments and participate in the out of hours on call organized by BEH MH Trust.

The Learning Disability Service provides 2 - adult and children inpatient Assessment and Treatment services at The Kingswood Centre. The services are for people with a Learning Disability who have mental illness and/or behaviour disorders so challenging that the local services are unable to meet their needs.
The Trust also employs Learning Disability Consultant Psychiatrists and medical staff in Kensington & Chelsea, Westminster, Hammersmith & Fulham, Brent, Enfield, Harrow, Hillingdon and Milton Keynes.

Clinical Work
- Provision of psychiatric expertise on mental health issues for people with Learning Disabilities, including providing specialist assessments and follow ups in the OPC and DVs, involvement in case discussions, advice, support and training to CTPLD and inpatient service colleagues where appropriate
- Regular liaison with mental health teams and general psychiatric units for patients who have been admitted to mental health services
- In conjunction with the other consultants, and junior medical staff respond to the needs of the service, including providing a rapid response to requests for pre-admission assessment (including for assessments under the MHA) when required for both patients from the Enfield catchment area and those referred from other Boroughs and services
- To provide appropriate referral to other tertiary and specialist services e.g. Forensic Services according to local protocols
- Participation in Care Quality and Audit program
- Providing cover for Consultant colleagues as agreed with the Clinical Director and Lead Consulta
Clinical Audit and Quality Improvement
- The post-holder is expected to actively participate in the Directorates Clinical Governance, Quality Improvement and Audit program.
Liaison with Trust/Council services and relevant non-statutory organisations:
- To liaise and consult with other professionals within the Enfield LD Community team
- To liaise with providers of services for people with learning disabilities including referrers, Commissioners, independent providers and carers where appropriate.
- To work collaboratively with colleagues from all disciplines within the Trust/Council and external stakeholders.
- To collaborate with GPs and clinicians from mental health adult services and SCAN/CAMHS as appropriate.
- To liaise with acute trust management to ensure up-to-date arrangements that support good practice
- To work collaboratively and actively with colleagues in other Learning Disability Psychiatry Teams in CNWL, sharing expertise and learning
- Provision of advice and consultation to general adult wards when people with learning disabilities are admitted
- To attend service user reviews, clinical, service-related, clinical governance and multidisciplinary meetings as necessary
Management
- The post holder will be professionally managed by the CNWL Learning Disability Clinical Lead and be professionally accountable to CNWL Medical Director. The community service is managed by Enfield Council and so overall patient responsibilities lie with the Council as per Section 75 agreement. Liaison, on call work and junior doctor on call work is almost all carried out in services run by BEH MH Trust except when it is in acute medical wards Chase Farm/North Middlesex hospitals when patient welfare is the responsibility of the acute Hospital Trust
- Private sector providers are responsible for providing accurate information as to the well-being of their clients and providing care that demonstrates benevolence, an awareness of basic physical and mental health awareness of relevance to people with learning disabilities, and a willingness to seek and respond to appropriate health advice. Safeguarding concerns are all managed by Enfield Council
- To be involved in service development and joint planning for adult learning disabilities services including expert advice, and guidance on the implementation of the Care Programme Approach (CPA) within the service.
