The clinicians will support a defined caseload of service users living in the community who have a range of mental health needs, utilising their specialist nursing/ OT / SW skills to help improve the health inequalities of our caseload population.

Main duties of the job
Undertaking required assessments of mental health, physical health and social wellbeing status and needs.
Offering a range of appropriate short and longer term, evidence based, health interventions for clients with complex , severe and enduring mental illnesses (SMI) providing interventions and working within the DIALOG+ framework.
Working collaboratively with the MDT to identifying health and social care needs, refer or liaise with Adult Social Care to address social issues like accommodation, finances, accessing education, training or employment and the need for additional care in the home (in compliance with the Care Act 2014).
Engaging our service user and their family in relapse prevention of their health and wellbeing, crisis planning, provide, or coordinate interventions that maximise the service users' ability to resolve crises, remain at home, avoid the need for hospitalisation and also safe stepping down of patients ready discharge.

We have recently transformed to the new Hub model and therefore it is an exciting opportunity to join the team when services are changing to be more responsive to the service users within the Hillingdon borough.
We are a multidisciplinary service, comprising of mental health nurses, occupational therapists, psychiatrists, psychologists, social workers, employment specialists and peer support workers. The team is supported by a good administrative system and offers an excellent work environment. The team has good links with local statutory and non-statutory services.

Developing and maintain effective working relationships and liaising effectively with colleagues within health services, GP surgeries, the local authority, voluntary agencies and other partner agencies to ensure a well-co-ordinated care, using excellent communication skills to develop and maintain therapeutic relationships with service users ,their families and carers who may at times be in situations of emotional distress.
