Job summary

Are you looking for high performing role in an exciting and supportive team? Home 1st Twilight nursing services are looking to welcome new colleagues to our team.

The successful candidate will use their experience of community nursing to support patient's and the established team in providing excellent levels of care which the Twilight Team pride themselves on.

As a qualified nurse you will provide clinical triage to all patient's cared for, ensuring the most appropriate care is provided.

In this role you will be expected to use your nursing skills to care for patient's in the Community and Care Home setting.

We work collaboratively with members of the multidisciplinary teams aiming to ensure appropriate treatment is available to all patients in their home, provided out of hours.

Main duties of the job

  • Provides skilled nursing intervention/care to patients of any age or condition
  • Undertakes delegated duties for the caseload holder under the supervision of the Twilight Senior Nurse on duty who is covering the
  • Takes responsibility for the caseload in the absence of the Twilight Senior Nurse including visits/assessments, workload management and allocation, dealing with referrals,
  • Maintains close liaison with the District Nursing teams, GP's, Urgent Care Centre, Urgent Community Response and other members of the primary care team and wider health and social care
  • Applies current Infection Control Policy issues and universal
  • Encourages patients towards self care and
  • Maintains up to date knowledge of methods of
  • Demonstrates, teaches and supervises the nursing care that families/carers can safely

Ensures that adequate medical supplies, equipment and other materials are maintained.
