Twilight and Overnight Service Community Nurse Positions - Band 5 RGNWe are looking for 2 to 5 highly motivated and enthusiastic nurses to join our dynamic team to provide high quality integrated patient care within Camden community services, in partnership with our local hospitals, GPs and other agencies/professionals as appropriate.Twilight and Overnight service is an extension of the day team, based at the St Pancras Hospital, 4 St Pancras way NW1 0PE.Post holder will be expected to cover the Twilight shift of 17:00 to 11:00Band 5 nurses areexpected to be kind and responsive but professional, informative and contribute to the quality ofthe services we provide by:? keeping the people who using our services as safe as possible through the use of soundclinical skills and effective risk assessments? ensuring the best possible clinical outcomes by using up-to-date skills and adhering toevidence based policies and procedures? ensuring the people using our services have a good experience by respecting, empoweringand working in partnership with people throughout the care planning process

Main duties of the job

Our aim is to be the best we can, offering excellent person-centred, safe and Evidenced based care to those who access our services.

You must hold a relevant Nursing Qualification with significant experience as a staff nurse. You should have some experience within acute care and/or community based setting, basic knowledge of relevant legislation and treatment methods used in this specialist field, be a car driver.You will:

  • perform joint visits to provide nursing care to patients allocated by a senior staff member
  • take responsibility for the assessment, management and evaluation of evidence based nursing care to patients in a variety of community settings, and promote effective teamwork within the wider Health Care Team.

The post consist of 2-5 x 6 hour shifts per week flexible.

The contracted hours will be 12 to 30 hours per week.

Essential

  • -Registered nurse (Adult) or awaiting pin, diploma or degree.
  • -Numeracy skills required for the safe administration of medicines
  • -Evidence of ongoing relevant CPD.
  • -Willingness to undertake further studies as required as part of this role.

Desirable

  • -Community experience as a student or substantive role.

Knowledge

Essential

  • Able to communicate with the multi-disciplinary team, use handover effectively, speaking face to face but and by telephone, writing letters and reports.
  • Able to maintain electronic patient records and working knowledge of Microsoft Office.
  • Care planning: being able to assess patients, develop, implement and evaluate a care plan
  • Empowering and working in partnership
  • Safeguarding and advocacy
  • Understanding how the Mental Capacity Act and Deprivation of Liberty Safeguards impacts on their work and the people they are working with
  • Understanding the concept of quality and their role in improving safety, clinical outcomes and the patient experience
