Job summary
You will be working as part of the Stroke Therapy Team. The team is made up of Physiotherapists, Occupational therapists, Speech and language therapists,Assistant practitioners and a Team administrator, all of whom are dedicated and passionate to provide outstanding care to stroke survivors. Thesuccessful applicant will also be passionate about Stroke and have experience working in health and social care, experience working with patients and skills and behaviours suitable for the post.The role when working across both acute and community pathways, is to support the assessment and treatment of stroke survivors, to assist withdischarge planning, family/carer communication, attending daily/weekly MDTs, providing goal-led rehabilitation and liaison with the wider MDT.
You will be a motivated individual, keen to implement change and empower staff to be the best they can be.
Main duties of the job
Our Furness Stroke Therapy Team currently provides a rotation between acute inpatient Stroke and community Stroke rehabilitation - allowing staff to be a part of the whole process from admission to discharge and ongoing rehabilitation in the community. The Furness team covers an extensive areaincluding Furness, Millom and South Lakes. The service is currently under development and will eventually include Saturday and Sundayworking. You will require access to a car.
The stroke team are supportive and committed to provide a friendly and flexible attitude which is patient focused and dedicated to quality care.Our aim is to ensure all our patients have access to appropriate treatment in the right location at the right time. Our approach to stroke services will require adegree of flexibility as well as creative thinking to establish and form new pathways of care. You will be providing care across the pathway withinthe acute (ward) and community settings (patients home/other settings/clinics).
About us
We operatefrom three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, andUlverstonCommunity Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies,daycase and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
