Job summary
We are looking for a doctor to join our exciting new programme in Furness - The CHIEF Programme - as Clinical Lead. We are developing a new multidisciplinary team to provide dedicated support for those patients who are at high-risk of admission to hospital and those who are recurrently being admitted.
The programme aims to be Co-ordinated, Holistic, Intensive, Early and Focussed, bringing together colleagues from medicine, nursing, pharmacy, social care, voluntary sector, therapy and community teams.
Working in an MDT environment you will provide clinical input and leadership to the team as you collectively define a management plan which addresses the needs of the whole patient across physical, mental and social care needs.
Clinical Leadership
This new programme will require strong and innovative clinical leadership. The successful applicant will need to develop strong relationships with the teams involved in the MDT and wider system partners. This is an opportunity to shape an exciting new model of care in Furness.
The Clinical Lead will also have responsibility for line managing clinicians working within the programme, ensuring high quality clinical care and participating in trust quality and safety work.
Main duties of the job
Working in an MDT environment you will provide clinical input and leadership to the team as you collectively define a management plan which addresses the needs of the whole patient across physical, mental and social care needs.
Doctors within the CHIEF programme will provide leadership of MDT meetings, clinical reviews (either remotely or face to face via home visits or clinic appointments) and support to the wider team with senior decision making.
Our data identified a very small number of patients (<1000) who drove ~30% of our admissions and bed days at Furness General Hospital. With your input we want to see the CHIEF programme address the diverse, unmet needs in these patients and then support them to 'live well' in their home environment.
The team will work during the working week with no evening or weekend work required, MDT meetings will be held throughout the week and are based in Barrow town centre.
About us
We operatefrom three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, andUlverstonCommunity Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies,daycase and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
