An exciting opportunity has arisen within the Care Home Support Team of University Hospitals of Morecambe Bay NHS Foundation Trust for a Band 6 Physiotherapist, to contribute with the multi-disciplinary Team in supporting the enhanced care in care homes program in line with local strategies.
We are looking for a Physiotherapist to join the North Lancashire Care Home support team based at Devitre House
The service works closely with a wide variety of the MDT to support the management of patients within care homes to proactively prevent hospital admission and effective case management
Main duties of the job
As a Care Home Support Team we are dedicated to the delivery of high quality, person-centred care centred care for frail / older adults being cared for within a registered nursing home setting. We work in partnership with the care homes to improve quality of life and care optimisation of residents, reduce avoidable adverse events and avoidable admissions to hospital / transfers of care, improve access to NHS services and provide professional support and mentorship to Care Home staff.
Our work engages with NHS, Primary care, and Adult social care and community services.
Successful candidates will demonstrate a passion for working with older adults presenting with frailty, partnership working and a commitment to providing high quality, compassionate, clinical expertise to care home staff in the management of their residents' care needs.
Working together to deliver care to patients you will need to:

About us
MBHT operates from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies,daycase and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

- Health and Care Professions Council registered Physiotherapist
- Honours Degree or equivalent in Physiotherapy approved by the Chartered Society of Physiotherapy/Health and Care Professions Council
- Evidence of relevant post graduate specialist clinical education

- Membership of the Chartered Society of Physiotherapy
- Cross prescriber

- Comprehensive rotational experience as a Staff grade in the core competencies within physiotherapy
- Evidence of effective written and oral communication skills in English in a variety of settings
