Clinical Lead - Community Health Monitoring Team

Job summary

We are looking to appoint Band 6 Clinical Lead to work within the Community health monitoring team. This is an exciting opportunity to work in a supportive and collaborative team.

The Post Holder will be required to work as an integral part of the team. flexible working will be required to meet the needs of the service and patients and will hold 24 hour delegated responsibility for the team caseload. Whilst working with other health care professionals the post holder will develop the ethos of integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.

Full UK driving licence essential.

Main duties of the job

You will work alongside the Team Leader and other Clinical Leaders providing expert advice and guidance to junior team members. You will actively manage the caseload of patients and undertake visits and support the management of staff within the team.

The post holder's role will be required to:

  • Clinical leadership and management of the clinical caseload for the team
  • Be responsible for the allocation, assessment, planning, implementation and evaluation of programmes of care without direct supervision
  • Demonstrate procedures to and supervise unqualified staff
  • Maintain and ensure high standards and continuity of care on a 24 hour basis
  • To support the movement of patients between the community in-patient beds, secondary care and community teams where appropriate.

About us

We operate from three main hospitals -Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
