Family Support Worker - Complications of Excess Weight service

Job summary

An exciting new opportunity has arisen for an enthusiastic, friendly individual to work within the paediatric Complications of Excess Weight (CEW) service. This service supports children and young people aged 2-17 with extreme high body weights and often with related health complications. This is an exciting and busy role which involves supporting children, young people and their families to work towards healthier lifestyle changes to improve their physical and mental health and wellbeing through 1-1 sessions, group working and organising events and activities to promote healthy changes.

The family support workers work collaboratively with the Multi-disciplinary team which includes consultants, dietitian, nurse, psychologists and physiotherapists to provide holistic care.

Main duties of the job

  • Excellent communication skills workingdirectly with patientsand families/carers intheCEW servicehelping them to adopt healthy lifestyle changes and to follow advice given by the doctors and healthcare professionals.Help families and clients to formulate individual care plans

  • Strong organisational and time management skills tohelp with the efficient running of theCEW service both clinically and administratively.

  • Liaisonwith the relevant teams to ensure a seamless service including GP practices,Schools and colleges.Other health care professionals and MDT'sPatients,familiesand carersAll agencies, groups and health professionals within South Tees who are involved in the delivery of the wider public health and health inequalities agenda.
