Job summary
A unique and exciting opportunity to join our team at our Primary care hospital wards in Redcar and East Cleveland (Brotton).
Zetland ward at Redcar Primary Care hospital is a rehabilitation ward for stroke and older persons medicine.
The post holders will work together to develop the therapeutic care programme for patients on the wards, working closely with the multi disciplinary team.
We are looking for caring and supportive candidates to provide one-to-one care for patients who require additional therapeutic support during their inpatient stay whilst also coordinating patient group activities and events on the ward.
The post holder will be an active member of the multidisciplinary team, using excellent communication skills to help maintain patient safety and dignity.

Main duties of the job
Work effectively as a member of the clinical team, carrying out designated patient care,activities, working etc.
Providing therapeutic care to patients in a group or 1:1 setting to minimise social isolation,
help prevent de-conditioning and help the patient to achieve their functional goals required before discharge.
Provide enhanced observation for patients assessed and cared for under CG47 level 3 observations.
Take an active role in discussing how a therapeutic care environment can be improved and nurtured with patients, carers and other team members.
Support the multi-disciplinary team in the delivery of high quality individualised patient care contributing to that care within the boundaries of his/her own role.
Demonstrate an understanding of how patient/carer feedback benefits and influences the service and participate in obtaining this feedback, communicating it to colleagues and responding appropriately.
To be actively engaged in the collection of Friends and Family data
Understand the importance of and keep accurate, timely and complete records consistent with Trust policies and procedures.
