Job summary
South Tees Hospitals NHS Foundation Trust is seeking to appoint an enthusiastic, motivated individual to join their integrated community therapy service partnership with Redcar and Cleveland Council, providing residential reablement and rehabilitation services to the people of Redcar & Cleveland.
The 7 day service is committed to the delivery of high quality client-centred care using evidence-based practice, aiming both to facilitate hospital discharge and prevent avoidable admissions to hospital or long term care.
As a Band 4 Therapy Assistant Practitioner within the Trust the postholder will also be part of the wider Integrated OT service. This is a unique partnership between Health and Social care in South Tees with a commitment to working together, improving the quality of our services and getting rid of existing barriers between health and social services.
Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.
Main duties of the job
Based in the community, you will work in the Redcar and Cleveland Meadowgate Centre.You will be involved with clients with a range of multiple medical and functional problems. The role involves carrying own independent caseload of patients including assessment and treatment planning. It also includes post-discharge follow-up with patients in their own homes, therefore driving or having access to transport is an essential part of this job.

You will be hard-working, resourceful and resilient. Continuing professional development is actively encouraged and regular support and supervision will be provided by qualified therapists. Participation in service developments and clinical governance is also expected, for which there is good support throughout the Integrated Therapy service to help you develop.
