An Excellent opportunity has arisen within Middlesbrough, Redcar & Cleveland Community Services. We are looking for skilled and outstanding senior clinicians to join our Urgent Community Response (UCR) service within the Growing the Friarage and Community Collaborative.
Consideration will be given to candidates who do not possess the level 7 advanced physical assessment skills qualification, however must possess the level 6 equivalent and be willing to work towards within the first 2 years of appointment .
Non-Medical Prescribing is essential.

You will have significant post-registration experience, be a non-medical prescriber and have advanced clinical skills to help you effectively manage a caseload of patients with highly complex needs. You will utilise your advanced clinical skills, non-medical prescribing and case management approach which will have a direct and positive impact on the person's ability to cope with an acute presentation with the aim to reduce unnecessary hospital admissions.
Experience of working in a community setting, working within an emergency assessment unit or rehabilitation would be an advantage and we welcome informal visits to the areas.
Main duties of the job
In the role of the Community Clinical Nurse Practitioner you will be a key member of the unplanned Urgent Community Response Service providing an in depth assessment of nursing care and treatment for patients in their own home to prevent unnecessary admissions and/or facilitate timely discharge from hospital.
Hours of operation is 24 hours per day over 7 days per week within the UCR service.
The successful candidate(s) will join a team of UCR Clinical Practitioners with a wealth of clinical knowledge and experience who will support and encourage you in your new post.
Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.
