Lead Structural Heart Specialist Nurse

Job summary

This role is being advertised as Band 8a Lead Structural Heart Specialist Nurse, however, applications from candidates who do not meet all of the essential criteria at this level will be considered for a Band 7-8a development role.

If you are appointed as a Band 7 Structural Heart Specialist Nurse, you will follow a training programme to become a Band 8a Lead Structural Heart Specialist Nurse. Please ensure you view both job descriptions/person specifications attached to the vacancy before applying.

The postholder will lead, develop and maintain the implementation of a fully comprehensive nurse facilitated structural heart service within the Tertiary centre, incorporating new technologies as they arise. The role will develop complex regional pathways of care for patients requiring consideration/work up for structural heart procedures. The structural heart nurse will be accessible as the first point of contact to key clinicians from within the Trust, and local referring centres who provide services for this specific group of patients.

Main duties of the job

The role is based in the acute Trust and will cover multiple clinical areas including wards, day unit, catheter labs and out patient. The role directly impacts on patient care throughout the total patient episode.

This will encompass:

  • Managing referrals - elective and urgent cases;
  • MDT - triaging to appropriate meeting, ensuring relevant information at hand and MDT agenda - weekly x2.
  • Coordination of the outcome of all MDTs - even those who are not accepted
  • Preadmission/admission - full clinical assessment and examination and adjustment of medical therapy according to clinical condition.
  • Daily clinical review and formulation of plan of care inclusive of discharge planning
  • Ordering and interpretation of appropriate complex tests
  • Non-medical prescribing
  • Delegated consent - of multiple procedures
  • Nurse facilitated discharge of patients undergoing structural heart procedures
  • Nurse facilitated follow up
  • This includes the development and initiation of work up and procedural pathways for a variety of structural heart procedures. The pathway will be developed in a way that avoids unnecessary delays and be reflective of current and developing data
