An exciting opportunity has arisen for a Band 4 Therapy Assistant Practitioner to join our friendly and dynamic Urgent Community Response (UCR) Home First Team at Redcar Primary care Hospital.

The post holder will be working under the guidance of the Specialist Allied Health professionals and will take responsibility for designated elements of the assessment, therapy intervention and the discharge for patients on the home first pathway of care and within the wider UCR service.

The post holder will have the ability to work independently, carrying out assessment and interventions for a specified and designated clinical caseload identified by a qualified clinician but also be able to work as part of a team supporting the clinicians with patient interventions.

The post holder will be expected to work flexibly to support the seven day rota between the hours of 8am-10pm.

Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.

Main duties of the job

You will have the ability to work under pressure, be well organised, demonstrate excellent communication skills and be able to work as an effective member of a multidisciplinary team.

The team is friendly and supportive and mentorship for this role will be available from the Clinical Service Lead for both UCR and Hospital at Home, Specialist Therapy Leads and the wider clinical UCR therapy workforce. We have a comprehensive training programme and our staff are encouraged and supported in their continual professional development through regular workshops and clinical supervision

Essential

  • English and Mathematics at key skills level 2 or equivalent.
  • Foundation Degree in Health and Social Care OR Equivalent Level 3 Qualification OR Level 3 Apprenticeship in relevant clinical care. OR Health and Science T-Level AND Equivalent knowledge and experience in a range of clinical care duties equivalent to Level 3

  • Substantial previous experience as an AHP Band 3 or equivalent therapy in Social Care
  • Evidence of achieving the KSF exit competencies of Band 3 Therapy Assistant.

Desirable

  • Experience of working in the community setting.

  • Excellent interpersonal, communication and active listening skills.
  • Basic IT skills to use electronic patient records and other Trust systems.
