Job summary
This exciting role has been developed to support the urgent community response multidisciplinary team working across the South Tees locality.
You will need to be able to work on your own; as part of a team and be involved with individuals with a range of medical, social and mental health conditions, therefore knowledge and experience of this is needed. You will work within agreed protocols and carry out therapeutic interventions and care programmes under the supervision of qualified therapists and nurses.
Applicants must have access to a vehicle for work purposes to meet the requirements of the role.

This is an opportunity for anyone with previous experience working within nursing, therapy or in care settings to help bridge the gap between hospital and home. As a senior integrated support assistant you will provide both rehabilitation and nursing care following a person centred plan within the patients' own home.
