Job summary

Paediatric Complications of Excessive Weight

Family Support Worker (band 3)

22.5 hours per week

The NHS Long-Term Plan has a key focus on improving care for children and young people with severe obesity. As part of the Children and Young People's Transformation Programme we are establishing a new service for the treatment and support of complications related to excess weight (CEW).

We are seeking a dynamic Family Support Worker to join a newly formed multidisciplinary team designed to deliver holistic care to patients who are severely obese.

You should have excellent communication skills, including the proven ability to motivate patients using a wide variety of approaches. Above all you must be passionate about working with Children and Young People, using drive and innovation to make positive changes for this specific patient group

You will be responsible for supporting the health and wellbeing of children and young people referred to the service and providing support to assist them in achieving their health goals.

Main duties of the job

  • Work with the CEW Multidisciplinary team and Administration team to deliver an effective service for our clients
  • To support the CEW team management of a designated case load within appropriate clinical and professional guidelines .
  • Use a range of verbal and non-verbal communication tools to communicate effectively with patients, carers and health care professionals in a variety of situations on different levels.
  • To take appropriate action in relation to risks and complaints, including reporting patient or staff safety concerns.
  • To travel independently between sites within the locality
  • To help people who are referred to the CEW service to manage health complications and lose weight and maintain weight loss.
  • To interact with clients both individually and in group situations.
  • To contact other stakeholders as required to provide or to request more information e.g. GP surgeries.
  • Promote the equality and value the diversity of individuals
  • Build up and maintain knowledge of contacts within the community

About us

South Tees Hospitals NHS Foundation Trust offers leadership and improvement training to all new staff to the Trust; this training programme has been designed to support our leaders in developing their understanding of leadership and management skills. You will be expected to attend the New and Aspiring leaders programme, and the Foundation Quality Improvement Programme that the Trust delivers. After you complete this one and a half day of training you will be able to explore further leadership and improvement training opportunities, we offer further in-house courses and bitesize programmes and leadership apprenticeships at level 3,5 and 7.
