Job summary

A unique and exciting opportunity to join our team at our primary care hospital wards in Redcar and East Cleveland (Brotton)

We are looking for caring and supportive candidates to provide one-to-one care for patients who require additional therapeutic support during their inpatient stay.

The post holder will be an active member of the multidisciplinary team, using excellent communication skills to help maintain patient safety and dignity.

Main duties of the job

Provide a full range of therapeutic care and interaction to patients whilst maintaining respect for the individuality, confidentiality and dignity of our patients. The role is underpinned by the Chief Nursing Officers 6Cs

Provide enhanced observation for patients assessed and cared for under CG47 level 3 observations

The post holder will be an active member of the multi-disciplinary team demonstrating a range of nursing skills to meet the needs of the patient in a safe, caring environment that promotes dignity, privacy and respect, under appropriate supervision

Use appropriate communication skills to communicate effectively and professionally with a wide range of people, recognising and managing barriers to communication.

Understand the importance of early identification of changes in patient behaviour and instigate a plan of care.

Recognise and respect patient confidentiality (unless the patient is at risk to themselves or others when concerns should be escalated to a senior member of staff)

About us

Zetland ward at Redcar Primary Care hospital is an rehabilitation ward for stroke and older persons medicine.

Tocketts ward at East Cleveland Hospital in Brotton is also a rehabilitation ward for older persons medicine and GP step up patients.

The post holders will work together to develop the therapeutic care programme for patients on the wards, working closely with the multi disciplinary team.
