Community based to review adult patients with Diabetes by telephone home visit & clinicTo teach DESMOND education programme from various venuesTo provide education for all Health Care professionals involved in diabetes

Main duties of the job

Do you have what it takes to join a dynamic expanding diabetes service? An Exciting opportunity have arisen for the right Specialist Nurse to join us and strengthen our Community Diabetes team. The successful applicants will have a passion for Diabetes and delivering care closer to home.

The post holder will lead the assessment, planning, implementation and evaluation of patient care in consultation with the MDT and other organisations to address individual needs involving patients and their carers in disease management, whilst encouraging patient participation in care and service development.

You will be the involved in the initiation and facilitation of research projects, clinical audit, national audit and national peer review of the service in order to demonstrate clinically effective practice.

You will establish, support and continue to maintain nurse and dietetic led clinics to support patients with diabetes.
