Exciting opportunity for someone with current or previous patient flow coordinator experience to join the Transfer of Care hub to work alongside our discharge facilitators in supporting patients and staff with complex discharges.

Main duties of the job

  • Identify patients for community hospitals
  • Welfare calls
  • Update systems used throughout the trust as required
  • Check patient referrals via the webice system
  • Be a point of contact for the home first team
  • Help and assist discharge facilitators as and when required
  • Support diagnostic virtual ward
  • Be a point of contact for care home staff regarding patients post discharge
  • Case manage and co-work complex discharge cases

Knowledge

Essential

  • Knowledge and experience of providing person centred care in a health and social care environment
  • Demonstrate relevant knowledge linked to how patients flow through the organisation and around potential discharge blocks (e.g. DTOCs)
  • Competent with software programmes including Microsoft Work, Outlook, Excel and Power Point
  • Experience working as a patient flow coordinator

Desirable

  • Knowledge of current processes
  • Knowledge of social issues and trends impacting upon hospital discharge

Qualifications

Essential

  • Educated to A level or equivalent. GCSE Grade C/4 or higher in English
  • NVQ Level 4 in a Healthcare, Customer Service or Business and Administration or equivalent/equivalent level of knowledge and skills
