Job summary

Shift work 4 days on 4 off 8am - 4pm over an 8 week rolling shift pattern. Part time hours maybe considered within these working hours / shift patterns.

The Reablement and Home First Team provide vital short-term services to people by helping them to regain or attain as much independence as they can in their own homes. The therapy led team works to: resettle people after a hospital stay and with people to identify and work on goals that will help them to live well at home

Main duties of the job

Senior Rehabilitation Assistants work with our Occupational Therapists, Physiotherapists and Reablement Coordinators to undertake assessments and provide review to ensure people are receiving the right level of input and / or are working on their outcome based goals.

Senior Rehabilitation Assistants work as part of a close team undertaking activities according to people's individual goal plans, encouraging them to do as much as possible for themselves, to live well and safely in their own homes. They also work with the Reablement coordinators to support and maintain quality standards of work delivered by the Rehabilitation Assistants.

Your role will involve completing assessments, determining risks and how to manage them, communicating and recording your feedback to others involved in people's rehabilitation.

You will need to hold a valid driving licence and have use of a vehicle to enable you to drive around the locality so you can work with people in their homes. Team working and communication skills are essential. You will be confident with handling IT systems to update written notes and electronic records.
