Job summary

Gloucester ICT Community Nursing Team are looking for an enthusiastic and dynamic individual to support the team in delivery of patient care in their own homes.

Community experience is essential with the ability to demonstrate a full range of community competencies. You will support and lead a team whilst caring for a complex and diverse caseload, working closely with your peers, wider integrated community team and GP practice staff. The role will entail triage in the referral centre (usually 2-3 times a month and a late shift (usually 1 a week) .

Our shifts are 8-4pm, 12-8pm- with one weekend a month on a rolling rota (days in lieu given).

Main duties of the job

  • To enable patients to live well within their communities, with a focus on physical health needs and wider holistic needs.
  • Lead Multidisciplinary Team meetings meeting as required- including GP supported clinical meetings (Gold Standards Framework or Case Management reviews)
  • Communicate and liaise with the Multidisciplinary ICT, Gloucestershire Care Services (GCS) and Gloucestershire County Council, Gloucestershire Hospitals NHS Foundation Trust as well as other referring acute and community hospitals
  • Discuss patient's conditions with them as appropriate and relay accurately and contemporaneously back to appropriate colleagues.
