Job summary

Complex Care at Home is an innovative service working to promote patient centred, preventative and proactive support for adults living with long term conditions, living in their own homes. We have a rare vacancy for a Dietitian to join our friendly, forward thinking team. You will join a multi-disciplinary team including nurses, therapists, health and well being coordinators, geriatricians, adult social care and housing colleagues, who are working to provide first class care to keep patients well at home. You will be visiting people in their own homes and community venues in Gloucester, Cheltenham and Forest of Dean.

You will have a passion for nutrition in adults with long terms conditions who may be frail and/or have dementia or other cognitive impairment. With a willingness to work autonomously in a changing environment, you will be encouraged to develop your dietetic knowledge and bring new understanding and skills back to the wider team.

Professional support will be provided by the therapy lead in the team and by the network of dietitians within the trust. Regular dietetic supervision, nutritional updates and sharing of local and national policy will be available. You will also be providing placement experience for dietetic students and you will be part of their ongoing training.

Main duties of the job

Working autonomously as the Dietician for the Complex Care at Home Team offering support to patients across Cheltenham, Gloucester and Forest of Dean localities.

Work as part of the MDT to provide dietetic knowledge and support to patients to help achieve personalised goals.
