Job summary
This is an opportunity to join the Mental Health Integrated Discharge Hub and become part of a team supporting patients from admission to discharge and beyond. It is an exciting time to join the integrated discharge hub team as many new initiatives to support the discharge process are being developed and trialled. The successful post holder will be integral in the development of these initiatives.
We are seeking a hardworking, enthusiastic and committed person, who has a keen eye for detail. The successful candidate would be expected to facilitate and support the management of bed allocation and discharge across GHC Mental Health and Learning Disability Trust Inpatient Services in line with Trust procedures and national guidance.
Good interpersonal skills are essential and the successful candidate will have had previous experience within a Mental Health or Learning Disability inpatient setting or Hospital or community team with an understanding of mental health issues.
Main duties of the job
The successful candidate will display the ability to communicate with a variety of professionals in order to aid patient discharges. The post holder must have the ability to multi-task, prioritise and problem solve in a calm, focused manner. Given the nature of the environments, the post holder may be required to adapt their day accordingly at short notice, which could then impact on scheduled arrangements.
The post holder must be available to work weekends to support the clinical on call manager with the allocation of beds out of ordinary working hours.
Flexible working is both supported and encouraged and at times necessary to meet patient needs.
This role would best suit a self-motivated, hardworking and driven individual who enjoys a challenge.
