Job summary
The Band 3 Community Health Care Worker is required to have a broad range of experience when working with People living with a learning disability; Ensuring reasonable adjustment within the General health care setting both in the community and inpatient areas
They will work for at team that strives to strengthen the NHS contribution to the prevention of, and tackling health inequalities within acute, primary, secondary and community care, working in partnership with services to make reasonable adjustments; a legal requirement to make sure health services are accessible to all disabled people.
The overarching aim is to ensure equal opportunities in health care for people living with a learning disability. The team operates a county wide service within Gloucestershire.
Main duties of the job
Under the supervision of the Registered clinicians Community Health care assistants will support implementation of care plans and provide information for assessments.
This will include supporting patients with all elements of the nursing process and be able to assist the Registered clinicians in the on-going assessment, planning, management and evaluation of care.
They will be expected to perform and record clinical observations and undertake clinical tasks appropriate to competence and setting, for example, venepuncture, ECGs and feedback findings to Registered clinicians.
Ensuring the privacy, dignity and safety of patient is maintained at all times recognising any issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care professionals. A very important part of the role is the ability to plan and organise one's own activity, prioritising workload appropriately in order to make the best use of time and feeding back to the team as appropriate.
They will be asked to input all activity with service users onto RIO or equivalent data in order to enable the collection of accurate statistics, as required by the Trust.

As a Health Care Assistant/ Support Worker you will be responsible for participating and assisting in the delivery of care / services to patients and service users as part of a Mental Health and Social Care Health Service. Health Care Assistant, Social Care Workers and other Healthcare Support Workers employed by the Trust are required to follow the guidance set out in Code of Conduct for Health Care Assistants/ Health Care Support Workers to provide safe and compassionate care of a high standard, and to challenge others who are not.
You will receive direct supervision and partake in reflective practice / training on a regular basis and be required to meet the Care Certificate standards. All successful applicants will also have the opportunity to discuss the prospect of undertaking a Level 3 Healthcare Support Worker Apprenticeship alongside their substantive post. This apprenticeship would include the Level 3 Diploma in Clinical Healthcare.
