Working in a small team made up of Nursing, Psychology, Occupational Therapy & administration.
Working in direct partnership with P3 Housing services, Substance Misuse services & Adult Social care to work intensively with a set caseload of individuals experiencing complex needs associated with homelessness.
Main duties of the job
- Provide assessment and implementation of evidence-based interventions for service users who are experiencing common, acute mental health and longer-term recovery needs
- Optimise the service users' ability to engage with Health & Social Care services and Housing and Homelessness services
- Support development of new team
- Work intensively with a small caseload alongside Psychology, Occupational Therapy and Administration colleagues.
- Work in direct partnership with representatives from Homeless Outreach, Social Care and Substance Misuse services
