Job summary
- To provide a specialist service for people with dementia and delirium under the supervision of the Consultant Admiral Nurse (Nurse Specialist for Dementia)
- To provide advice and support to patients, their families and other healthcare professionals under the supervision of the Consultant Admiral Nurse.
- To ensure continuity of a high standard of evidence based care, supporting the needs of patients, their families and other carers affected by dementia/ delirium by initiating appropriate steps for effective care.
- To be actively involved in patient care, liaising with ward staff to ensure high quality care this includes supporting the Butterfly Scheme
- To maintain mechanism for referrals to the Admiral Nurse and Dementia Practitioner, this will include carryout any initial assessment and refereeing complex cases to the Admiral Nurse
- Provide support as needed to the Consultant Admiral Nurse, teams and ward teams across the trust

To further develop service and initiatives working with the Consultant Admiral Nurse we are seeking a highly motivated individual, who is committed to improving the quality of life for people living with dementia and their family/carers who use services at East Surrey Hospital.
The post holder will ensure continuity of a high standard care, supporting the needs of patients, their families and other carers affected by dementia/ delirium by initiating appropriate steps for effective care as well as being available for initial assessment and refereeing complex cases to the Consultant Admiral Nurse.
