Job summary
This is a great development opportunity to join us as a Band 5 Community Forensic Practitioner with a plan to develop you to a Band 6 in 12 months.

To provide specialist assessment and treatment for people with mental disorder, who have offended or are at risk of offending. The post holder will have a key role in the care coordination of patients within the community setting as well as Forensic assessment for patients in the community and in-patient setting

Main duties of the job
Band 5
- Responsibility of care-coordinating a caseload of patients within the communitysetting
- Ensuring patients' needs are considered through a bio-psychosocial approach
- Work effectively with the multi-disciplinary team to ensure safe and effective care and treatment is provided
- Promote recovery by actively supporting and facilitating independence and social inclusion
- Ongoing management and monitoring of risk
- Administration of prescribed medication in the community setting
- Build, develop and maintain positive relationships with statutory and voluntary agencies relevant to the role
- Undertake mental health assessments in a variety of settings such as psychiatric hospitals and community settings to assist in the formulation of risk management
- To offer support and advice to client's carers and relatives as appropriate
- Provide written and/or oral reports to agencies as appropriate
- Promote the understanding of mental health and related issues to other agencies through liaison and education
- Liaise with any professionals working with the individual including MAPPA, Social Supervisor, VISOR etc
- Provide both formal and informal clinical supervision to Community Support Workers
- Be a resource for specialist advice and information to other colleagues, agencies, organisations and people who use the service
