Senior Community Children's Learning Disability Nurse

Job summary

The Children and Young People's Learning Disability Service (CLD), part of the Learning Community within the Mindworks contract at Surrey and Borders Partnership NHS Foundation Trust (SABP), is seeking a highly motivated and skilled Band 7 Nurse to join our team based in Godalming.

CLD is a Surrey-wide, nurse-led community service. Our multidisciplinary team includes Learning Disability Nurses, Assistant Psychologists, Psychiatrists, Music Therapists, and our dedicated administrative staff.

We offer twice-monthly Clinical Issues Meetings, monthly Business Meetings, and a quarterly Learning Disability Forum. Regular supervision and appraisal are provided, alongside ongoing opportunities for career development.

As a Band 7 Nurse, you will be responsible for providing supervision and appraisals for Band 6 colleagues, as well as offering clinical advice and case management support.

You will hold a group of allocated young people and will work with them and their network to support their presenting need.

CLD operates a specialized pathway for ASD, ADHD, and behavioural assessments, and you will play an integral role in supporting the assessment process, conducting observations, and gathering histories for young people.

To act as practice assessor/teacher in teaching, facilitating and supporting pre-registration students and junior personnel and to participate in the audit of educational placements

Main duties of the job

  • To demonstrate highly specialist nursing knowledge and skills in areas associated with learning disability such as crisis intervention, alternative communication, the assessment and management of people with additional mental health needs, epilepsy, medication management, challenging behaviour and other conditions.
  • To actively involve children and families in developing and agreeing their own care plan,
  • To act as an advocate and ensure that there is proper regard for their dignity, choice, self-esteem, racial, cultural, sexual and religious needs and preferences.
  • To practice within the Positive Behaviour Support framework.
  • To undertake comprehensive risk assessments and develop effective risk management plans.
  • To work in partnership with acute and primary care services and other healthcare providers.
  • Establishing and maintaining effective relationships with colleagues, other agencies, children and young people who use services and their families.
