Working within Mindworks Surrey gives clinicians and practitioners the opportunity to really consider the best needs for a child/young person's emotional wellbeing and mental health needs, based on the iThrive framework. As a developing clinician, you'll get to understand the breadth of quality and evidence based interventions available across Surrey, from those very specialist clinical services to our therapeutic groups, counselling offers and school based services, all provided by NHS and our third sector partners.

Working within one of our 3 Hubs (Chertsey, Guildford and Leatherhead), you will be supported by caring, professional colleagues. You will work in base to continue the excellent collaborate working and problem solving that happens in real time. You will use your skills to screen new requests for support and in collaboration with our alliance partners to direct and signpost young people to appropriate service. You will record clinical decisions using our record system, use your organisation skills to manage flow through the screening and triage pathway and be able to recognise risk that needs to be prioritised and acted upon in a timely manner.

Main duties of the job

To provide timely screening and triage of children and young people where Mindworks has received a request for support for their emotional wellbeing/mental health needs, including offering discovery conversations where ensuring the safety of the child/young person may be a priority.

To work individually and jointly within the Mindworks Surrey alliance to support children and young people be able to access the most appropriate care for their needs.

To confidently signpost using the iThrive framework, considering where needs may not be based in mental health and where support can be sought from a range of Surrey systems partners.

Job description

Job responsibilities

The post holder will be integral to the functioning of the Mindworks Surrey Hubs, responding to requests for support for emotional wellbeing /mental health needs.

The post holder will work as an autonomous practitioner, undertaking clinical screening and where appropriate, assessment, of children and young people who may have a range of needs, including more complex co-morbid mental health needs or may need urgent support. The post holder will work alongside medical, nursing and other professional colleagues to ensure that the most effective care and treatment is provided resulting positive outcomes for that child/young person.

To co-ordinate and liaise with the Mindworks Hub team to ensure flow of new requests are appropriately discussed and considered.

To work within a highly experienced multi-disciplinary team involved in the assessment and treatment of children and young people with a range of mental health difficulties.

To manage a caseload as part of the Mindworks Hub team, from receipt of request to transfer to other services or closure, ensuring there is appropriate information to enable and inform decision making.

Ensure that outcome measures are considered, embedded and audited. Contribute to relevant research and ensure that interventions on the pathway are evidence based. Raise concerns and gaps in pathways to the Service Manager to enable planning and continuous improvement of flow and accessibility.

Other duties:

To accurately record all activity as per Professional and Directorate standards, including risk summaries, risk assessment and care planning. To confidently formulate a robust My Safety Plan with young people and their families where required.

Contribute to the development and implementation of clinical guidelines, protocols relevant to sphere of responsibility as well as continuous improvement initiatives.

When demand and capacity allows, to support team members to meet the demands of the service.
