Dementia Clinical Nurse Specialist

Job summary

My job makes lives better by providing specialised care for people admitted to the Meadows in-patient dementia services as part of a multi-disciplinary team that offers mental health and psychological support to the person, their family/carers and friends.

This is a new post being introduced in the service and is suitable for Registered Mental Health Nurses/Practitioners with experience in dementia care. The successful applicant will have the knowledge, compassion and a strong sense of advocacy to co-ordinate the delivery of person and therapeutic relationship centered dementia care in the following key areas:

Formulating tailored care plans based on the person's individual needs and incorporating pharmacological and other treatment plans

Taking a lead in supporting the nursing team by advising on the care of people on the ward experiencing behavioral and psychological symptoms of dementia (BPSD) and young onset dementia.

Clinical leadership with policy development, advice, education and consultancy to other health and social care professionals as well as quality improvements within the service in relation to dementia care.

Helping friends/families/carers to navigate their way through available services and support.

Main duties of the job

  • Work collaboratively with the multi-disciplinary team and other professionals to assess, treat and improve the identification and management of co morbidities and frailty for people with dementia and their carers/families.
  • Provide a range of bio-psycho-social interventions to promote health and well-being for people with dementia and their carers/families
  • Develop and support care plans for people with dementia and families including positive risk management
  • Support people with dementia, carers/families and staff to understand and respond to changes in behaviour and relationships as a result of dementia
  • To actively engage people with Dementia and Cognitive impairment and their carers in all stages on intervention, ensuring that their views are acknowledged and considered.
  • Work in partnership with colleagues to promote the use of advance care planning for people with dementia and to support and inform families, around advance care planning, best interest decisions.
  • To liaise and work closely with linked Social Workers to facilitate timely provision of social service input and community mental health including formulation of safe discharge plans.
  • Ensure that clinical records are up to date and maintained and stored appropriately in line with local and national GDPR regulations.
