Community Psychiatric Nurse(s)
Guildford and Waverley Community Mental Health Services for Older People (CMHTOP) are seeking an experienced and enthusiastic community mental health nurses to join the team. To work within a community team based within area working in partnership with statutory and voluntary sector to facilitate whole system approach to care delivery. The Post Holder will act as Care Coordinator/Lead Professional, where appropriate, to patients who require the skills of a Mental Health professional, and provide a holistic, clinical autonomous nursing service, to enable individuals to engage in the best standards of care available.
The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role.
Main duties of the job
You will be required to hold lead and professional responsibility for a number of service users. This reflects a need to demonstrate excellent communication skills and be able to work collaboratively with a multidisciplinary team. You will be committed to the delivery of a high-quality service, working both independently and as part of a well-established, innovative and recovery-focused team in meeting the needs of service users and their carers.
In addition to holding a caseload, the successful applicant will be offering initial assessments to clients referred to the service and be supporting individuals in mental health in the community. There are various training opportunities to build skills in the delivery of low intensity psychological interventions.
