Band 6 - Learning Disability Acute Liaison Nurse
30 hours per week
The SABP Learning Disability Acute and Primary Liaison Service are looking to recruit a Nurse to join the current team.
We welcome applicants who are RNLD, RMN or RGN qualified with learning disability experience. The post is hosted by SABP but works with our acute partners.
We are seeking applicants who are enthusiastic, enjoy partnership working and can work independently.
The successful applicant will work alongside the existing Senior Learning Disability Acute Liaison Nurse based at East Surrey Hospital in Redhill.
You will be provided support to develop your nursing skills within an established team, to enhance your autonomous practice and engage with a range of different agencies. The Trust actively encourages professional development through the appraisal and supervision process and access to training to develop skills and competencies. You will be provided with regular supervision.
The Learning Disability Acute and Primary Liaison Service provides liaison to the acute hospitals in Surrey and GP practices in Surrey Heartlands ICB and Frimley ICB, South.
The applicant must have a valid driving license and access to a vehicle for work. Some travel is required to other parts of the county on SABP business so you will need to be comfortable travelling between locations.
Main duties of the job
The role is suitable for a dynamic and compassionate nurse who is committed to supporting the needs of adults with a learning disability who are accessing the services within an acute hospital setting either as an inpatient, outpatient or planned admission.
You will need to be passionate about ensuring a high-quality, holistic approach to the health care and support provided to people with learning disabilities in the acute care environment. You will be required to work autonomously and as part of a wider team and contribute to a range of projects
The role includes:
Holistically Assessing, planning implementing and evaluating delivery of nursing interventions.
Supporting people with LD and their carers when accessing the hospital services, delivering a specialist nursing advice and liaison service. This may include providing advice regarding mental capacity, consent and best interest to the person, their family/carer/other professionals.
Training acute colleagues regarding the needs of adults with a LD.
Providing supervision and contribute to appraisal of junior team members and supporting induction of new team members.
Role as practice supervisor/assessor for student nurses
Partnership working with a range of health colleagues and agencies.
