Job summary
The Hope Service is our flagship for the integrated provision of high intensity services for children and young people between 11 to 18 years of age in Surrey. The collaborative approach by social care, health and education is unique and this is a great opportunity for you to be the clinical lead for a team of nurses within this brilliant service.
You can have your main base in either Epsom or Guildford however, there will be an expectation you will spend time on both sites. This is a community role so as well as attending the bases there will be opportunity for you to visit our Acute Hospitals, Schools and homes of our young people as part of the assessment, care and treatment approach.
The Hope Service offers outreach and a Day Programme during office hours. The nurses and social workers are care coordinators for the young people who use the service and you will be expected to hold a caseload and lead assessments. As Clinical Lead you would have oversight of the daily planning for the nurses ensuring safe and effective clinical delivery is being undertaken. Your clinical expertise and leadership will support the General Manager and Team Leaders for the service to have a focus on excellent nursing practice and alongside the Nurse Consultant you will be an advocate for the health team and how they can have their own professional identities whilst also being part of the multi agency, multi professional group.
This is a senior clinical and leadership role and not operational
Main duties of the job
The team undertaken assessments in acute hospitals, the Health Based Place of Safety, schools, young peoples homes. There is a need to ensure clinical oversight of the nursing team regards distribution of work and ensuring safe practice such as Lone Working. As Clinical Lead you would provide this coordination and work closely with the General Manager and the Nurse Consultant regarding the Health Team. The Health Team consist mainly of nurses, alongside Art Therapists, Drama Therapists, Psychologists and Consultant Psychiatrists. You would also develop your relationship with the Senior Social Worker as there is connectivity and interdependency between our health, social care and educational staff to meet the needs of the young people and the families/carers using our service.
There are various offers for training and development - all staff working within the crisis pathway for young people are offered external training regarding assessment and risk assessment of adolescents and for DBT-A. Alongside this you may wish to consider training in other areas such as Advance Nurse Practitioner courses and you will have clinical supervision with the Nurse Consultant for the Service who can develop with you your personalised development plan as part of your induction, supervision and appraisal processes.
