Job summary

The Hope Service is our flagship for the integrated provision of high intensity services for children and young people between 11 to 18 years of age in Surrey. The collaborative approach by social care, health and education is unique and this is a great opportunity for you to be the clinical lead for a team of nurses within this brilliant service.

You can have your main base in either Epsom or Guildford however, there will be an expectation you will spend time on both sites. This is a community role so as well as attending the bases there will be opportunity for you to visit our Acute Hospitals, Schools and homes of our young people as part of the assessment, care and treatment approach.

The Hope Service offers outreach and a Day Programme during office hours. The nurses and social workers are care coordinators for the young people who use the service and you will be expected to hold a caseload and lead assessments. As Clinical Lead you would have oversight of the daily planning for the nurses ensuring safe and effective clinical delivery is being undertaken. Your clinical expertise and leadership will support the General Manager and Team Leaders for the service to have a focus on excellent nursing practice and alongside the Nurse Consultant you will be an advocate for the health team and how they can have their own professional identities whilst also being part of the multi agency, multi professional group.

This is a senior clinical and leadership role and not operational

Main duties of the job

The team undertaken assessments in acute hospitals, the Health Based Place of Safety, schools, young peoples homes. There is a need to ensure clinical oversight of the nursing team regards distribution of work and ensuring safe practice such as Lone Working. As Clinical Lead you would provide this coordination and work closely with the General Manager and the Nurse Consultant regarding the Health Team. The Health Team consist mainly of nurses, alongside Art Therapists, Drama Therapists, Psychologists and Consultant Psychiatrists. You would also develop your relationship with the Senior Social Worker as there is connectivity and interdependency between our health, social care and educational staff to meet the needs of the young people and the families/carers using our service.

There are various offers for training and development - all staff working within the crisis pathway for young people are offered external training regarding assessment and risk assessment of adolescents and for DBT-A. Alongside this you may wish to consider training in other areas such as Advance Nurse Practitioner courses and you will have clinical supervision with the Nurse Consultant for the Service who can develop with you your personalised development plan as part of your induction, supervision and appraisal processes.

Job responsibilities

You will assist the Hope Management and Leadership Team in the co-ordination and risk management of the CYP receiving care from the Hope service.

Liaise with the Hope Team Managers to monitor staff sickness/absences and take appropriate action as per Trust policy, and ensure appropriate cover is organised.

To lead, organise and participate in multi professional meetings, chairing meetings as required and allocating referrals and tasks as appropriate to team members.

Monitor the service caseload and provide accurate and timely information on the number of CYP in the service including those admitted to Tier 4 (hospital), length of stay and outcomes and staff activity.

Work in partnership with the Surrey Heartlands T4 Provider Collaborative regarding CYP at risk of hospital admission or who, following admission, require discharge planning between services to ensure quality of inpatient admission and timely discharge back to home and community.

Responsible for ensuring a timely response to referrers and the delivery of comprehensive care packages to CYP and families/carers, ensuring all referrals are processed, allocated and information entered into the appropriate information system/s.

Liaise with Hope Managers to effectively manage the service within agreed standards, targets, time-tables and constraints. This includes the support to developing our teams to understand the principals of Thrive and how we can all work collectively across Surrey to meet the needs of CYP.

To establish an effective communication system between all professionals in the local service, by attending / chairing staff meetings, Team Briefings, Case Reviews and other relevant meetings.

Ensure student nurse placements are offered and are of a quality to ensure learning and practice is developed. Work with Head of Nursing and Nurse Consultants for CYPS regarding links with University and ensuring partnership working between the organisations is effective.

Deputise for General Manager alongside the Team Managers as and when required.

Provide training for other agencies around risk management / Self -harm and mental health. This includes our Emergency Services eg Police and training material/courses they may require.

Deliver presentations at a local and national level.

Support management of caseloads, ensuring that the nursing team are providing care plans, risk assessments and My Safety Plans to all CYP and families/carers allocated to them and reviewing anyone on a CPA in accordance with the national standards.

Participate in joint assessments with the community CYPS clinicians, Assessment Suites, inpatient units and other agencies.

Establish within the multi-professional team and Day Programme a shared understanding of CYPs needs, care plan and discharge arrangements, to ensure a safe, supported and effective transition between the Service and original referring team, or other appropriate care, identifying the contributions from health and other professional into the CYPs plan.

Facilitate and organise care plan approach meetings [CPA] for referrals from inpatient units back into the community involving all agencies who may be providing care, support and treatment.

Provide consultation to other professionals working within Mindworks Surrey Teams and Partner agencies. Establish a good working relationship with CYPS Division, Mindworks Alliance Partners and other agencies to determine best plan of care for the young people and their families and how best to work together.

Lead in provision of supervision and appraisals for staff you lead maintaining standards in accordance with own disciplines code of conduct and the Trust Supervision policy, Clinical Governance and Quality Standards.

Support investigation into any clinical incidents, ensuring Datix is completed and that training and development identified is undertaken.

Accountable for own professional actions and conduct as per NMC guidance and the Trust standards, interprets policies for the good of the people we serve.

As a member of the management and leadership team, actively support and lead service preparation for Foundation Standards Review, CQC and/or Ofsted.

Work with Scc colleagues and SABP PALS department regarding any formal complaint. Where there is an informal complaint work with your service and Division to address concerns raised and work with complainant to resolve the matter where possible.

Participate in the Quality Operational Board for CYPS to ensure information and discussions are brought to those platforms and that you in turn discuss this with your team in business meetings.
