Job summary
As an integral and highly valued member of the multi-disciplinary team, the Peer Support Worker (PSW) will provide formalised peer support and practical assistance to service users in order for them to regain control over their lives and their own unique recovery journey. Through sharing the wisdom of own lived experience, the PSW will inspire hope and belief that recovery is possible in others.Within a relationship of mutuality, they facilitate and support information sharing to promote choice, self-determination and opportunities for the fulfilment of socially valued roles and connection to local communities.
Main duties of the job
The PSW will provide formalised peer support and self-management assistance to service users to help promote recovery and prevent relapse for people being discharged from Home Treatment Team services following a period of mental health crisis.
This will involve offering up to 10 sessions of peer support to service users.In their role, PSWs will support service users in using a self-management booklet to develop a personal recovery plan, including steps to move on following a crisis, plans for keeping well, planning responses to signs of a future crisis developing and how to achieve goals and ambitions.

Within a relationship of mutuality, they facilitate and support information sharing to promote choice, self-determination and opportunities for the fulfilment of socially valued roles and connection to local communities.
The PSW will work under the supervision of line management within the Peer Support Services. The PSW will also attend specific training in delivering formalised peer support and ongoing supervision with an appropriate Trust clinician. The PSW will also be responsible for providing records regarding their activity as required for the Peer Support Services.
