As a Learning Disability Acute Liaison Nurse you will work in partnership with the acute hospital trust and other healthcare providers within the Trust boundaries. You will establish and maintain effective relationships with people who use service and their carers/families, with colleagues & other agencies,

You will deliver a specialist Learning Disability nursing advice and liaison service. The role focusses on actively assisting adults with learning disabilities, as well as their carers/families when accessing the acute services. Assessing patient's needs, making recommendations, and providing support from pre-admission to discharge. Carrying a time limited case load and acting as a point of contact.

The role also includes:

  • Promoting the inclusion of adults with learning disabilities within patient support services and carers' groups. To ensure active participation with the review of hospital services.
  • Participating in appropriate meetings within the Trust
  • Promoting and maintain a "Champions" network within the Hospital.
  • Providing guidance regarding Mental Capacity, Consent, Best Interest to the service user, their family/carer, and relevant professionals.
  • Provide routine reports as appropriate
  • Participating in regular supervision to receive support and have time to reflect upon practice.
  • Providing learning disability awareness education and training.

Main duties of the job

The SABP Learning Disability Acute and Primary Liaison Service are looking to recruit a Nurse.We welcome applicants who are RNLD, RMN or RGN qualified with learning disability experience.The post is hosted by SABP but works with our acute partners. We are seeking applicants who are enthusiastic, enjoy partnership work and can work independently. The successful applicant will work with the Senior Learning Disability Acute Liaison Nurses based at Royal Surrey Hospital Guildford and St Peters Hospital Chertsey, this is a split base role. Support will be provided to develop your skills in an established team, to enhance autonomous practice and engage with a range of partners. The Trust actively encourages professional development through appraisal and supervision, access to training to develop skills and competencies. You will be provided with regular supervision.

The Service provides liaison to the acute hospitals in Surrey and GP practices in Surrey Heartlands ICB and Frimley ICB, South. The applicant must have a driving license and access to a vehicle for work. Travel is required to both hospitals and other parts of the county on SABP business, you will need to be comfortable with travel.

The post is within the Acute and Primary Care Liaison Team, which is part of the specialist learning disability health care services. The post holder will be based within the Acute Hospitals and expected to travel County wide as and when required.
