Job summary

A unique and exciting opportunity has arisen for a highly motivated, enthusiastic individual to join our dynamic multi-disciplinary community team as a Clinical Health Care Assistant.

As a Clinical Health Care Assistant (CHA) within the East Surrey Community Team for People with Learning Disabilities (CTPLD), you will work as an integral member of a dynamic multi-disciplinary team working closely alongside a range of professional clinicians.

The post holder will have experience working with people with a learning disability, be able to manage and prioritise a caseload whilst supporting implementation of clinician care plans to a high standard as developed by experienced clinicians including Nurses, Occupational Therapist, Dietitians, Physiotherapist, Speech and Language Therapists. The post holder will be required to work autonomously whilst demonstrating good interpersonal skils, including verbal and written.

Main duties of the job

This post requires elements of practical work - including but not limited to : teaching to maximise independence, assistance with practical health issues - supporting health appointments, provision of advice, development of accessible information. The post holder will, in addition be required to support physiotherapy which involves water-based therapy and, the successful candidate will be supported to access relevant life saver training.

The post holder will be required to engage with relevant training opportunities, supervision, and will be supported by the clinical team in determining the opportunities available.

The team covers the East of Surrey, with bases at both Redhill and Leatherhead & Epsom. There will be an expectation for some travel between bases. The successful applicant will have a clean driving licence and a vehicle that can be used for work purposes.
