Job summary
We are seeking an experienced and motivated individual to join a supportive forward-thinking urology cancer nursing team at Ashford & St peter's Hospitals
The post holder will lead on the delivery of high quality, patient centred care for men living with and beyond cancer.
The post focuses on providing expert clinical, psychological and educational support to patients on the prostate cancer PSFU surveillance pathway, ensuring safe, efficient and holistic management through out their cancer journey.
You post holder will work autonomously and collaboratively with the wider MDT, supporting patients who have completed active treatment and are transitioning to a self management pathway.

Main duties of the job
- Act as the key clinical contact for men on the PSFU pathway.
- Monitor patients remotely through PSA surveillance, identifying and acting promptly on abnormal results or patient concerns.
- Deliver nurse-led clinics to include HNA , treatment summary and discharge clinics.
- Provide expert advice to patients, families and healthcare professionals regarding treatment side effects, symptom management and holistic needs.
- Contribute to evaluation and ongoing service development of the PSFU surveillance pathway in accordance with national standards.
- Work in partnership with CNS colleagues, cancer support workers, urologists & oncologists to ensure seamless care transitions
- To be a resource both within the trust and the community for clinical advice and information.
- With the Lead Nurse contribute to clinical governance within the trust for cancer patients.
