Job summary
We are recruiting for a B4 Rehabilitation Assistant in Early Supported Discharge for Stroke.
The role is community-based, supporting patients with their rehabilitation at home. The caseload is complex and varied, and you'll visit patients with physical, cognitive and visual impairments. You'll work closely with our multidisciplinary team to provide individually tailored rehabilitation programmes.
The majority of work is directly with patients in their homes, sometimes using a combination of methods including digital platforms and telephone as well as face to face. Flexibility plus good organisational and prioritisation skills are essential.
This is a good opportunity If you are interested in developing greater skills in neurology and rehabilitation, and would enjoy the chance to carry your own caseload - completing assessments with a specified list of patients and formulating plans for them. You need NHS experience in stroke rehabilitation/neurology and have preferably worked in the community.
A valid UK driving license, and access to a car with business insurance is essential.
Main duties of the job
Undertake skilled support work to aid assessment, treatment, and management of stroke patients. This includes gathering collateral information and providing intervention within set parameters to facilitate patients reaching an optimum level of independence / social integration
Work independently, as part of the ESD team carrying a delegated caseload within set parameters andunder the guidance and supervision of qualified staff
Take responsibility for time management, planning and organisation of delegated workload on daily basis
Report on patient changes/ progress in a timely manner
Assist in making onward referrals
Maintain effective communication with patients, carers and all relevant professionals
Undertake non-clinical duties e.g. ordering of equipment, recording contacts/stats
Assist in the implementation, evaluation and modification of ESD service
Help to provide a high quality, effective and efficient ESD service
Work autonomously, assessing risk, and judging when senior support is required
Continue professional development including participation in training programmes
Work closely with and liaise with partner organisations
There is an expectation that the post holder will be required to travel between sites for meetings/training as necessary and to support other teams if required
All clinical duties will be regulated by an HCPC registered Physiotherapist, Occupational Therapist, Speech & Language Therapist or NMC registered Nurse
