Job summary
Chestnut ward has an exciting opportunity for a forward thinking, proactive and highly motivated person in the role of a Patient Flow Coordinator. The Flow Coordinator is a vital part of the MDT and the post holder will; Co-ordinate all stages of the patient journey, from admission to discharge. This involves proactively supporting and facilitating the work of the Multi-disciplinary team in delivering the best outcomes for the patient; informing and communicating fully outcomes of assessments and discharge plans. The Flow Coordinator will work closely with the Nurse in Charge and CSNP team on a daily basis reporting to the NIC of the shift and accountable to the Ward Manager. The applicant should have an understanding of all varied processes involved in the transfer and discharge of patients and be able to confidently use the computer systems in place in the trust. The role requires the post holder to be able to prioritise their work load and must enjoy working as part of a team. The role covers an 8am - 4pm period at present and the post holder will be required to work Monday to Friday.
Informal visits to the unit are welcome so you can come and meet us and see the way that care is delivered.
Main duties of the job
o Work closely with and be responsible to the NIC of the unit toensure safe effective patient flow is implemented through the unit.o Closely communicate and work with the CSNP Team the FlowCoordinator and NIC on Chestnut ward. This role involves telephone and electroniccommunication with a variety of member from the MDT, thereforeexcellent communication skills are vital.
