Job summary

My name is Jay and I am the current Senior Pharmacy Homecare Services Practitioner. It has been a privilege to lead a dedicated, friendly, and hardworking team committed to delivering essential pharmacy and financial services in support of the wider pharmacy teams, and patients across OUH.This role presents a unique and rewarding opportunity to make a meaningful impact on patient care. You will help ensure the delivery of high-cost medicines and the safe treatment for patients in their homes. At the same time, the position offers valuable leadership experience and the opportunity to shape and nurture a compassionate, collaborative team.In my time in this role, I have been fortunate to collaborate with the clinical teams and the senior Homecare team to turn ideas into actionable improvements. Main duties of the job

  • To support the operational aspects of the pharmacy homecare service with planning, developing and organising, under the direction of Pharmacy Homecare Service Manager.
  • Co-ordinate the pharmacy homecare medicines service, prioritise and manage the workload and resolve problems as they arise alongside colleagues.
  • Liaise with homecare service providers to review and prepare service level agreements for implementation within the Trust.
  • Co-ordinate service review meetings for homecare medicines management working closely with homecare companies, pharmacists, nurses, doctors, other healthcare staff and patients.
  • Assist the Homecare Pharmacy Service Manager in maintaining and reporting of departmental Key Performance Indicators (KPIs).
  • Plan, manage and implement change in the homecare team to develop efficient and effective delivery of homecare services.
  • Responsible for the induction of new and rotational staff. Developing, updating and delivery of a suitable training package for homecare pharmacy staff.

The Pharmacy Homecare Services role is within an exciting and ever diversifying area to work in. We have a large team to support you with developing and improving our existing services and processes along with creating new processes to suit more complex services. This role will require working alongside a colleague in the same role with a clear definition of duties and responsibilities but with allowance for cross-cover for annual leave.

Team Mission Statement: The Pharmacy Homecare Team facilitate the delivery of medication to patients in their homes, by the most cost effective and efficient proven method. Improving patient choice, outcomes and optimising value for money.
