Job responsibilities

Clinical:

Demonstrate clinical expertise in the delivery of direct nursing care: co-ordinating and monitoring the care of transplant recipients, acting as a role model to the nursing and wider MDT and demonstrating partnership with patients and their carers. Coordinate the care of TPIAT patients pre and post procedure.

• Work clinically to optimize health and wellbeing of transplant recipients, demonstrate autonomous decision making related to comprehensive history taking and physical assessment of patients, follow up blood tests and other investigations.
• Provide consultation in nurse-led transplant follow-up clinics in association with a Consultant at the Churchill and at outreach centres and in multi-disciplinary clinics.

Ensure the effective delivery of a high standard of nursing care for all patients within the nursing team by providing clinical leadership, direction, supervision and maintaining effective systems of work.

• Provide triage and management of acute patient enquiries via telephone advice line.
• Provide an advice line for community practitioners and other healthcare workers.
• Act as advocate for individual patients.
• Practice a high standard of communication including highly sensitive and complex information to develop and sustain partnership working with individuals, groups, communities and agencies. Utilise evidence based information through a variety of communication media.
• Work collaboratively with band 7 colleagues within the post-transplant team to supervise and monitor nursing practice within the team to maintain standards and ensure competence.
• Promote a high standard of advanced specialist nursing by initiating and co-ordinating the assessment, planning, delivery and evaluation of the holistic needs of patients/families through evidence based practice following agreed policies, protocols and guidelines.

Participate in the transplant recipient co-ordination on-call, including working weekends and bank holidays.

Adhere to the NMC Code of Conduct at all times and practice within the NMC Scope of Professional Practice.

Management:

Proactively recognise and respond to factors which impinge upon staff morale and standards of care, liaising with the Deputy Matron as appropriate.

Facilitate regular and effective appraisals for the post-transplant band 6 team and support appropriate training and development opportunities in line with role requirements, departmental objectives.

Collaborate with the multidisciplinary team to develop a shared vision of the service.

Act as a role model and resource in expert clinical evidence based practice.

Work collaboratively with colleagues to develop a shared vision of the service and effective documentation, pathways, protocols and guidelines for care. Demonstrate a commitment to share expertise and disseminate information.

Participate in implementing national standards and frameworks at a local level.

Use effective change management skills to initiate and implement service and practice development, making the best use of resources to improve practice and health outcomes.

Have active involvement/lead where appropriate in the recruitment process including the selection and appointment of new staff

Maintain involvement and active input in multidisciplinary meetings and appropriate local bodies / forums to contribute to practice and service development

Education/Training:

Maintain, advance and develop personal theoretical knowledge, high standards of clinical competence and professional development through ongoing self-education and attendance at relevant courses of study.

Develop and maintain a peer network of support, information and learning with other nurse specialists at a local and National level.

Maintain own professional portfolio.

Contribute to the development of specialist nursing knowledge and skills to enhance and extend the practice of other nurses and health care professionals within the service.

Identify and address the educational needs of patients, families, carers and staff involved in the delivery of this service.

Contribute to the teaching and support of students from the multi professional team within relevant educational programmes.

Provide an appropriate learning environment, including mentorship, coaching and preceptorship.

Develop and maintain a peer network of support, information and learning with other nurse specialists at a local and National level.
